Spidajak is right in that sense, but he is missing a valid point of his social conten. He is speakin to himself as he spilling those secrets, telling his popular self that he doesn't need it to mask his inner self anymore. He's telling the mask that today that part of him dies and the real him lives. Hense the part " I know you know what's going on, girl. This is me without you.

The lyricman is right except the doctor part.
The girl is not the one hurt, but the guy is hurt because she spilled out his inner most darkest secretes to the world as it says in this part.
