    This song is about him (Joel) losing the love of his life. He misses the person and asks why did they have to die. He wishes that he had told them how much they had meant to him. He still thinks about them and feels guilty that he is moving on with out them. He still misses them enough that he still pays for their phone bill just to hear the sound of their voice. So to escape the pain Joel would start drinking. That's why the song is called Maybe Two or Twenty. He would start drinking until he became numb. He wishes that he could of saved the person he loved and that they were happy together.
