A guy is going out with the daughter of a member of the mafia, and is trying to break up with her - which is when she lets slip that the "small family business" that her family owns is, in fact, the mafia; if he breaks up with her, they'll kill him. He doesn't go for her offer of "there's still time"; the mafia are going to kill him, first "busting his kneecaps", which is very painful.

Johnny is going out with a girl who has family relations to the mafia. The lyrics "we've got a small family business" and "you call it crime, we call it smart family business" indicate this. The ex-girlfriend is threatening him and reminding him about the family connections to the mafia. "I'll tell uncle Rocco, to call off the guys, with the crow bars" implies that "bust your Kneecaps" is literal and not slang for "punish severely." It means that, should he break his girlfriends heart by dumping her, the mafia will murder him starting with busting his kneecaps, presumably with crowbars.
